There are four chords you need to know:
1) Em: first finger on the second fret of the D string,
pinky on the third fret of the high E
2) G: first finger on the fifth fret of the D string,
pinky on the seventh fret of the high E
3) C: first finger on the third fret of the A string,
pinky on the fifth fret of the B string
4) D: first finger on the fifth fret of the A string, ring
finger on the seventh fret of the G string, pinky on
the seventh fret of the B string
They appear in this order: Em, G, C, Em, C, Em, D, C and then repeat.
Ben plays them in a fingerstyle pattern using his thumb and first finger,
I personally prefer to use thumb, first and middle finger.
The pattern is always over four strings:
For the Em, your thumb plays the D string, then the first finger plays the B
string, middle plays high E, first plays B. (Always repeat once)
Slide up to G, pattern stays the same.
C: The pattern remains the same but is now one string lower. The thumb plays
the A string, first finger plays G string, middle finger plays B string.
This pattern is the same for D.

As I said, these same notes can be played in tons of different configurations
on the fretboard (this version just matches the one time you can see it being
played live). The last slide could also be a quick hammer-on/pull-off to the
fifth fret.
